Strengthening Multidisciplinary Care: Case Conferences.

Consultmed has launched Case Conferences, a secure, bidirectional collaboration thread embedded in every referral. 

Case conferences and multidisciplinary team (MDT) meetings are a well-established way to coordinate care for patients with complex or chronic conditions. They bring together GPs, specialists, allied health providers, and nursing staff to share information, align treatment plans, and clarify responsibilities. By embedding this capability directly into Consultmed’s smart referral workflow, receiving services can start a Case Conference with a single click, notify the referrer, and invite the right members of the care team into one secure thread. The result is active collaboration, less duplication, and better-coordinated care for patients.

How does the Case Conferencing feature work in Consultmed? 

Step 1: Patient is referred to the Cancer MDT Clinic using Consultmed’s smart customised referral.

Step 2: The MDT clinic reviews the referral and starts a Case Conference, opening secure two-way communication with the referrer and care team. The oncologist, radiologist, surgeon, genetic counsellor, breast care nurse, and referrer are invited. 

Step 3: The team collaborates in real time. Discussions and decisions are securely captured within the Case Conference.

Step 4: Discussions and agreed outcomes are captured and delivered through connected integrations. Best Practice and MD Clinical for GPs, and Gentu and MD Clinical for specialists. A date and time stamped PDF transcript is also available for clinical records and audit.
