Connecting Digital-First Weight Management with General Practice.

Not every new Consultmed partner looks like a hospital, health service or PHN.

Our collaboration with Eucalyptus is a first for Consultmed, working with a large-scale, digital-first healthcare provider to ensure virtual care integrates effectively with general practice.

Eucalyptus delivers condition-focused digital health services across Australia, including Juniper, a medically supervised weight management program. As demand for accessible telehealth services continues to grow, Eucalyptus recognised the
importance of strengthening communication with patients’ regular GPs.

The goal was to ensure digital care supports and strengths the role of general practice, rather than functioning separately from it.

The Challenge
Digital-first healthcare models have rapidly expanded access to care, but they can also creates gaps in communication between telehealth services and a patient’s regular GP.

For patients with complex or ongoing health needs, including those seeking weight management support, continuity of care is essential. Weight management often intersects with chronic disease, cardiovascular risk, mental health, and long-term medication use.

Without structured communication pathways, GPs may lack visibility into treatment decisions, medication prescribing, and patient progress.

Eucalyptus wanted a solution that would ensure their services remained well-connected to primary care, supporting safe and coordinated patient management.

The Solution
Through the partnership with Consultmed, Juniper is now available on the Consultmed platform, enabling GPs to refer patients and communicate with Juniper clinicians.

Using Consultmed, GPs, nurse practitioners, and other primary care providers can securely refer patients to Juniper directly from their practice management software.

Once a referral is made, the platform enables closed-loop communication, allowing clinical information to flow securely between providers.

This includes:
– Referral submissions from GPs to Juniper clinicians
– Structured clinical assessments and treatment plans
– Medication and prescribing decisions
– Follow-up updates and review outcomes
– Ongoing clinical correspondence between providers

All correspondence is delivered directly into the GP’s practice management system, creating a complete and auditable clinical record while ensuring the GP remains informed throughout the patient journey.

Strengthening Shared Care
This partnership supports a shared-care model where patients can benefit from the accessibility of digital services while maintaining the oversight and continuity provided by their regular GP.

Patients referred to Juniper can access medically supervised weight management support while their GP retains visibility over:
– Clinical assessments
– Prescribing decisions, including GLP-based therapies where appropriate
– Follow-up and review outcomes
– Ongoing management recommendations

This approach helps ensure that digital care enhances, rather than fragments, the patient’s overall healthcare experience.
